Discussed with the Provider that the Original background checks/reports are required to be on-site.

An unannounced renewal inspection was conducted with the Provider-Mrs. Torrico this visit took place in the morning between 10:00am-12:15 there were 7 children with the Provider and 1 assistant for a total of 32 points within the supervision guidelines. The children were observed having morning free play, working on gross motor skills, playing with blocks, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-90-A
Description: Based on observation of the children records, one child record did not include documentation that the child had been adequately immunized. Evidence: Child #7's record did not include documentation of a immunization record (ENROLLED 4/20/15).

Plan of Correction: Provider stated that the child has an appointment today and the parent will bring it tomorrow.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the physical plant, the diapering surface was found rip/torn and no longer nonabsorbent. Evidence: The diaper changing pad was being tapped together from rips/torn and no longer non absorbent.

Plan of Correction: Provider stated that she already ordered a new diaper changing pad.
