Today an unannounced renewal inspection was conducted with the Provider-Mrs. Leena Batavia. This visit took place in the morning, between 9:00am-11, there were 10 children present with the Provider and 2 assistants for a total of 30 points within the supervision guidelines. The children were observed being dropped off by parents, having morning breakfast and snack, story-time, free play, nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation of the infant bottles, two bottles were found without the date. Evidence: Child #2's bottles did not include the date. Plan of Correction: Provider dated the bottles during this inspection.
Standard #: 22VAC40-111-60-B Description: Based on observation of the children records, three children records were missing the following information parents employer address, 2 emergency contact name, address and phone number. Evidence: Child #1's record did not include the mom's work address. Child #2's record only had one emergency contact person listed. Child #3's record did not include mom's complete work address. Plan of Correction: Provider will notify the parent this evening and request the information.
