Comments:
An unannounced monitoring inspection was conducted on April 29, 2015. At the time of entrance there were 9 children, (a total of 19 points) in care, 1 provider and 1 assistant were also present. The sample size of records consisted of 2 children's and 2 household members records and 1 caregivers record. Two records from the previous inspection were also reviewed and violations were corrected. Children were observed coloring, singing, dancing, napping and playing educational games on the computer. No medications are being administered at this time. The inspection began at approximately 10:15 am and ended at 12:20 pm. The results of the inspection and the violations cited were reviewed and confirmed with the Provider. Areas of non-compliance are identified on the violation notice.

Violations:
Standard #: 22VAC40-111-940-A
Description: Based on observation and interview with the provider, the provider did not ensure temperatures were maintained at or below 40?F in refrigerator compartments and at or below 0?F in the freezer compartments. Evidence: During the inspection conducted on April 29, 2015, the temperature in the freezer exceeded the required O F (zero or below) temperature as the thermometer reading for the freezer was 20 F .

Plan of Correction: The provider will get a thermometer.

Standard #: 22VAC40-111-100-B
Description: Based on a record review, the provider did not obtain a copy of a physical examination within 30 days of the child's first day of attendance. Evidence: During the inspection conducted on April 29, 2015, the record for child #2 (first date of attendance 10-22-12), did not include a copy of a physical examination.

Plan of Correction: The provider will get the physical from the parent.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children. Evidence: During the inspection conducted on April 29, 2015, a container of hand sanitizer labeled "keep out of reach of children" and 'warning"was observed sitting on the bathroom counter and accessible to children.

Plan of Correction: The provider will remove the hand sanitizer.

Standard #: 22VAC40-111-320-G
Description: Based on an interview with the provider, the provider did not ensure that wood burning stoves and fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Documentation of the inspection and cleaning shall be maintained by the provider. Evidence: During the inspection conducted on April 29, 2015, there was not documentation available of an inspection conducted.

Plan of Correction: The provider will find the documentation and send to licensing.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the licensee did not ensure the diapering area was nonabsorbent. Evidence: During the inspection conducted on April 29, 2015, the diaper changing pad was observed and it has 5 small rips and therefore it is no longer nonabsorbent.

Plan of Correction: The provider replaced the diaper changing pad during inspection.
