Comments:
An unannounced monitoring inspection was conducted in the Family Day Home on April 30, 2015. Ten children were in care. The children's activities were supervised by the Provider and an Assistant. The ages of the children ranged from infancy to preschool. The total points were 24. The Provider reported ten (10) children are currently enrolled. The LI observed the children during free play, the lunch meal and nap time. The child care areas were supplied with age appropriate materials and toys for the children. A sample of records was reviewed. Documentation of the practiced emergency drills and the first aid kit were reviewed. A prescribed emergency medication was on hand for a child, the Provider and 4 Assistants have completed medication administration training (MAT). Thank you for your cooperation during the inspection. Standard #: 22VAC40-191-60-B
Description: Based on the LI's review of an Assistant's record, the Family Day Home Provider failed to maintain documentation of a completed sworn statement or affirmation in the employee's record. The date of hire was documented as 01/18/2015. Verified background checks were reviewed in the record.

Plan of Correction: The sworn statement was signed by the employee prior to employment. The document has been misplaced from the record. The individual will complete another sworn statement before the next scheduled work day.
