Technical Assistance:
Discussed with provider MAT training will send contact information and playground maintenance routine/ schedule for mulch

Comments:
An unannounced monitoring inspection was conducted on from 10:00 am- 11:00 am . There were 5 children present observed with the provider. The children were observed in a variety of activities to include: free play outside, a picnic snack and in transition from a nap inside to a walk around the neighborhood. The following areas were inspected during today?s visit: physical environment, emergency drills, records ( children and caregiver) emergency supplies and posted documents. Standard #: 22VAC40-111-60-B
Description: Based upon record reviewed, required information was found to be incomplete. Evidence: Child A's record had only 1 of 2 required emergency contact information complete.

Plan of Correction: The provider had highlighted information to be requested from parent. Verified that out of state contact was acceptable.
