54.1-3408 Provider must be MAT certified to administer prescription medication.

Technical Assistance:
Today the licensing inspector (LI) and the family day home provider discussed adjusting the policy for administering medication for each child to match the certification status of the provider. Currently there are no medications in the home and the provider's Medication Administration Training (MAT) has expired. The LI and provider discussed the time limits of each of the background checks, Tuberculosis screenings, and First Aid/CPR certifications. The LI and provider also discussed updating the immunization record of each infant every 6 months until 2 years of age.

Today an unannounced monitoring inspection was conducted from 10:00am until 11:30am. There were 4 children (14 points) with 1 provider. Children were observed practicing Spanish, working on number puzzles, coloring, having tummy time, napping, being changed, and being fed. Violations were observed and are on the violation notice form. The space is organized and clean. If you have any questions please contact me at or 703-554-4995.

Standard #: 22VAC40-111-60-A
Description: Based on record review, the provider did not maintain an up-to-date record for each child. Evidence: 1. Child #1 did not have complete proof of identification information and immunization record. Child Record was missing the date in which the original birth certificate was viewed, and who viewed it. Child #1 record was also missing the physical and immunization record. 2. Child #3 did not have complete proof of identification information. Child record was missing the date the original birth certificate was viewed.

Plan of Correction: Child #1 - form was reviewed and signed on 5/1/15. Immunization record was brought in on 5/1/15. Child #3 - Form was viewed and filed on 5/1/15

Standard #: 22VAC40-111-510-A
Description: Based on observation and interview an individual crib was not available for each child that requires a crib. Evidence: There are 2 cribs in the home and 3 children under 12 months of age that require an individual crib for napping or resting.

Plan of Correction: New crib was put in place on 5/4/15

Standard #: 22VAC40-111-690-D
Description: Based on observation the diapering surface is not nonabsorbent. Evidence; The changing pad in the day care is worn and has cracking around the edges.

Plan of Correction: New changing pad was added on 5/4/15

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review and interview with the provider, background checks have not had repeat checks before three years since the last dates of the last sworn statement, central registry findings, and criminal history record check. Evidence: 1. Criminal Record check was last completed 9/19/2011 2. Central Registry check was last completed 10/21/2011 3. Sworn Statement check was last completed 11/21/2011

Plan of Correction: Criminal Record was notarized on 5/2/15 and sent to VA State Police. Central Registry was notarized and and sent to VA state police. New Sworn Statement was completed on 5/3/15.
