Technical Assistance:
A supplemental page was provided. It is my understanding that you are not using the upper section of the large swing set or the yellow slide on the playground because they do not meet the licensing standards.

Thank you for your time and assistance with the unannounced monitoring inspection conducted on May 4, 2015. The on-site visit was from 10:55 a.m. to 1:25 p.m. On this date there were twelve children in the care of the provider and an assistant for twenty-two points. There were a total of fifteen children enrolled in the program. There were four children's records reviewed, and the records of the provider and one assistant were checked for updates. The provider, the assistant, and three children were interviewed. There were no medications being administered at the time of the visit, which is the policy of the provider. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations are noted on the supplemental page. The violations from the previous inspection were reviewed for compliance. Lunch was observed to meet USDA guidelines. The children were working hard on Mother's Day presents. Outdoor play, choice time, hand washing and bathroom procedures, lunch, and nap time were observed during the visit. Standard #: 22VAC40-111-920
Description: Based on observation of all posted information and discussion with the provider, the provider failed to post or give to parents the weekly menu of meals and snacks. Evidence: The rooms used for care were observed on 05/04/2015. 1. There was no meal/snack menu posted for the parents for the week of May 4th through May 8th. The provider normally posts the menus by the month but she stated her May menu was on the computer but she had not posted it.

Plan of Correction: The provider will post the menu for the month of May today.

Standard #: 22VAC40-111-60-A
Description: Based on review of children's records and verification from the provider, it was determined that the enrollment information for three children had not been updated within the last year. Evidence: There were four children's records reviewed during the 05/04/2015 inspection. 1. Child # 1 was enrolled on 10/29/2012 and the parent had reviewed and updated the enrollment information on 11/12/2013 but not since that time. 2. The date of enrollment for Child # 2 was 09/05/2013 and the enrollment information had not been updated since that time. 3. Child # 3 was enrolled on 07/19/2011 and the parent had reviewed and updated the enrollment information on 07/18/2012 but not since that time.

Plan of Correction: The provider will go through the records of all of children enrolled in the program and have the parents review and update all enrollment information. She will begin to do this annually.
