Comments:
An unannounced monitoring inspection was conducted today, May 7, 2015 to determine the provider's compliance with licensing standards. In attendance were 4 children totaling 8 points being supervised by the provider. Children were observed using the restroom in preparation for nap time. Children books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found. Due to lawn maintenance the outdoor play areas were not inspected. The family day home had the following posted: license, menu, emergency numbers, evacuation maps, daily schedule, previous inspection, and various parent information. These postings are located on the wall upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected. Two children and the provider records were inspected with the majority of the required information found. The following was also inspected: fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Description: Based on the inspection of provider records, the required background check was not conducted. Evidence: During the inspection of provider records, it was discovered the Criminal Record Check in the file dated 1/24/11 expired on 1/24/14. An updated background check was not in the record. Licensing standards require background checks to be submitted every three years from the original date of the initial background check.

Plan of Correction: Per provider: I will locate the background check previously submitted and place in the record. If I cannot locate the background check, I will call the agency to obtain a duplicate and place that in the record.
