Technical Assistance:
Reviewed standard 22 VAC 40-111-90.C (updated immunizations)

Licensing Inspector arrived at 11:45 am and departed at 1:00 pm. At the time of the inspection, there were 11 children in care (ages infant through preschool), for a total of 30 points. Children were observed napping. Infant care was also observed. No medication is being administered or stored on site.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, it was determined that the provider did not ensure that all child obtained documentation of obtaining a physical examination by or under the direction of a physician within 30 days after the first day of attendance. Evidence: 1. Child #1 (date of enrollment 11/13/14) did not have documentation of a completed physical examination on file. 2. Child #3 (date of enrollment 12/17/14) did not have documentation of a completed physical examination on file.

Plan of Correction: Provider will request that parents bring in copies of the physicals.

Standard #: 22VAC40-111-210-A
Description: Based on review of provider/assistant/substitute provider records, it was determined that the provider did not ensure that in addition to satisfactory completion of first aid training and CPR training, caregivers obtained a minimum of eight clock hours of training annually in areas relevant to their job responsibilities. Evidence: The record for caregiver #2 indicated that she had completed only eight hours of training in the past year.

Plan of Correction: Provider will ensure that all assistant and substitute providers complete 16 hours of training annually.
