Comments:
An unannounced monitoring inspection was conducted beginning at 12:50 p.m. and ending at 2:30 p.m. with the provider, assistant and provider's adult daughter. There were seven children in care ranging in age from five months to four years old for a total of eighteen points. Children were observed during nap time. Rest areas were observed. Play areas and supplies were observed. Four children's records and two staff records were reviewed. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review of four children's records, the provider failed to obtain documentation of a physical exam within 30 days of attendance for two children. Evidence: Child A, first day of attendance 3/10/2015 and Child B, first day of attendance 4/6/2015 did not have documentation of a physical in the record.

Plan of Correction: I have repeatedly asked Child A's parent for a copy of the physical. I will get this as soon as possible. I will get a copy of the physical from Child B's parent as soon as possible.

Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records, not all records were complete. Evidence: Child B, first date of attendance 4/6/2015, did not have the addresses for two emergency contacts.

Plan of Correction: I will get this information at pick up today.

Standard #: 22VAC40-111-70-A
Description: Based on review of four children's records, not all parents received required information prior to the first day of attendance. Evidence: Child B, first day of attendance 3/10/2015, did not have documentation of liability insurance disclosure.

Plan of Correction: I will get this information at pick up today.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of staff records, not all criminal history record check were updated as required. Evidence: Staff B's record did not contain an updated criminal history record as required. The most recent criminal history request was a county background check dated 6/18/2013.

Plan of Correction: The state criminal history record check will be completed for Staff B as soon as possible and sent.
