Comments:
An unannounced monitor inspection was conducted on 5/19/2015 from 9:48 am until 12:31 pm. Upon arrival, there were a total of ten (10) children ages 8 months to 5 years old, the Provider and one (1) assistant present. Children present totaled 31 points. Provider does not administer medication and does not transport children. Provider continues to participate in the USDA Child Care Food Program. Children and staff records were reviewed during the inspection including two (2) records for recently enrolled children & staff/household records were. Previous violations were reviewed and have been corrected. Children were observed during morning activities, afternoon nap, lunch served, arrival/departure procedures, individual/group activities and electronics/multi-media activities. Risk Assessment & areas of non compliance were reviewed with the Provider on 5/29/3025.

Standard #: 22VAC40-111-530-A
Description: Based on observations, the Licensee failed to have linens consisting of a top & bottom cover or a one-piece covering theat is open on three edges EVIDENCE: A top cover was not provided for 1 infant child. LI Anderson observed sleeping infant in a crib without cover.

Plan of Correction: Provider will add a cover to the child. Make sure each child is supplied with a top and bottom cover.
