The licensing inspector and provider discussed that two providers will need to update their Tuberculosis Screening in the month of June.
Today an unannounced monitoring inspection was initiated at 11:00am and completed at 12:45pm. There were 12 children(32 points) with 1 provider and 2 assistants present. Staff files, household files, children's records, and physical plant were observed. Children were observed having circle time, practicing colors in spanish, and getting ready for nap time. The home is colorful and has an abundance of toys readily available for the children. Infants have their own play space. Standard #: 22VAC40-111-180-A Description: Based on record review, 1 of 4 household files did not have a current report of Tuberculosis Screening updated every two years from the date of first screening. Evidence: Household Member #2 last had a Tuberculosis Screening Report on 7/19/2012 Plan of Correction: I will have the household member get the screening.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review, 1 of 4 household members did not have an updated Central Registry Check every three years since last check. Evidence: Household Member #2 last had an updated Central Registry Check on 12/29/2008 Plan of Correction: I will fill out the forms and send them out this week.
Standard #: 22VAC40-191-40-D-4-a Description: Based on record review, 1 of 4 household members did not have a completed Criminal History Record Check after turning 18 years of age. Evidence: Household member #2 is 21 years of age and does not have a criminal history record check documented in the file. Plan of Correction: I will fill out the form and turn it in this week.
Standard #: 22VAC40-191-40-D-5-a Description: Based on record review, 1 of 4 household member records did not contain a Child Protective Service Central Registry Check within 30 days of becoming 14 years of age. Evidence: Household member # 4 did not have documentation of a Child protective services central registry check. Plan of Correction: I will fill it out and send it in this week.
Standard #: 63.2-1809-A Description: Based on record review, 1 of 3 records did not contain complete documentation for proof of identification. Evidence: Child# 2 did not have the proof of identification information documented on the child's record form in the child file. Plan of Correction: I will talk with the parents and get the appropriate documentation.
