Technical Assistance:
The provider will need new MAT training, but she has no children on medication and will not give medication until it is renewed.

Comments:
Two children present with the provider during this renewal visit. Total points today is 4. The children are listening to music, cutting and gluing papers. Drawing and writing in their journals. The provider has lots of toys and different interest areas to explore. Interaction between the provider and children observed. Today's lunch is spinach and ham sandwich and applesauce with milk to drink. This visit was started at 10:20 and ended at 11:45. Description: Based on the review of two children's records, the provider failed to get proof of ID for one child in care. Evidence: Child A is missing proof of id.

Plan of Correction: I will ask the parents to bring the proof of ID so I can record the information in the file.
