Today an unannounced monitoring visit took place in the morning between 9:15-11:30 there were 5 children present with the Provider for a total of 15 points within the supervision guidelines. The children were observed having morning snack, free play, reading books and actively engaged in working on fine gross motor skills. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-120-A Description: Based on observation and review of the caregiver records, one record did not include two or more references were verified prior to employment. Evidence: Caregiver #1's record, only had one reference check. Plan of Correction: Provider stated she did not know she needed too and stated she will have another one completed.
Standard #: 22VAC40-191-40-D-1-b Description: Based on observation of the records, one caregiver record did not include documentation of a Central registry check and criminal record check within 30 days of employment. Evidence: Caregiver #2's record, did not include a Central registry check (8/14). Plan of Correction: Provider stated that she misplaced it and will have another one completed.
