An unannounced renewal inspection was conducted on 5/29/2015 from 9:00-10:45 a.m. At the time of entrance, there were five children in care with one provider. The age range of children was 6 months to 2 years, for a total of 15 points. The children were observed during music, circle, and AM snack times. The children were observed singing, playing with toy cars, and toileting. The provider does not transport children. The provider is MAT certified. Five children's records, one provider record, and two household members records were reviewed. Standard #: 22VAC40-111-500-C Description: Based on an observation of the outdoor play equipment, it was determined that the two slides that the children use are placed on a wooden deck. Evidence: The two slides that the children use are placed on the back wooden deck. Plan of Correction: The provider stated that she will search for a resilient surface to place under the two slides. In the meantime, she has removed the two slides from the access of the children.
