Discussed with provider: The need to obtain MAT training/ certification if any children in care require medication.
An unannounced monitoring inspection was conducted on from 9:30 am- 10:30 am. There were 7 children present observed with the provider . The children were observed in a variety of activities to include: arrival, free play, person hygiene and completing work sheets. The following areas were inspected during today?s visit: physical environment, emergency preparedness plan, emergency drills, records ( children and caregiver) emergency supplies and posted documents. Standard #: 22VAC40-111-830-C Description: Based on review of posted records, required documentation of emergency evacuation drills for April 2015 was found to be missing. Plan of Correction: Provider will ensure all required drills are documented on the drill log.
Standard #: 22VAC40-111-60-B Description: Based upon review of records, required information was found to be missing. Evidence: Child A's record was missing documentation of the annual file review completed by parent. last record of file review was 2/2/14. Plan of Correction: Provider will have all parents complete file review for the new school year in September.
