Nine children were in care during the inspection, total of14 points. The age range of the children was from two years to nine years old. Children were eating lunch, watched a movie and played with toys. New number for LI is 540-272-9214 Inspection time 11:45am-1:00pm

Description: Based on review the menu was not current for a one week period. The menu was not completed for today or the rest of the week.

Plan of Correction: I will compete the menu for a week.

Description: Based on review not all household members had a current T.B. test. Evidence: House hold member A did not have a T.B. test every two years. The late T.B. test is dated4/16/12.

Plan of Correction: I will have them take a T.B. test.

Description: Based on observation children's hands were not washed. Evidence: The children's hands were not washed after eating lunch.

Plan of Correction: We will wash after eating.
