An unannounced renewal study was conducted from 1:30 pm - 2:45 pm. There were 8 children in care ages, 6 months, 8 months, 8 months, 12 months, 23 months, 2 years, 3 years, and 5 years for a total of 25 points with the provider and one assistant. The children were observed napping and having free play. Lunch is provided from home. The most recent shelter in place drill was conducted 5/26/2015 and the most recent fire drill was conducted 5/3/2015. The provider, one household member, and one assistant records were reviewed. Eight children's records were reviewed. The provider is current in CPR, First Aid, and MAT (Medication Administration Training). The provider has 16 hours of annual training. Standard #: 22VAC40-111-60-B
Description: Based on review of 8 children's records it was determined that not all records were complete. Evidence: Child A and Child B were missing one of the two required emergency contacts.

Plan of Correction: The provider will request the missing emergency contact information from the parents of child A and child B.

Standard #: 22VAC40-111-180-A
Description: Based on review of one household member record it was determined that not all household members repeated TB (Tuberculosis) screening every two years. Evidence: The most recent TB screening on record for household member A is dated 01/08/2013.

Plan of Correction: Household member A will obtain a TB screening and submit the results to the provider.
