An unannounced monitoring inspection was conducted today. There are 3 children present (12 points) with one adult, including one of the provider's own children. 2 children's files were reviewed. See violation notice form missing information. Previous violations have been corrected. Children were observed resting, eating and playing. Description: Based upon direct observation, bottles are not labeled and/or dated.

Plan of Correction: I will ask the parents to labeled and date the bottles.

Standard #: 22VAC40-111-100-A
Description: Based upon a review of files, one child's file is incomplete. Evidence: Child A does not have a completed physical on file, past 30 days of enrollment. (Date of enrollment: 5/4/15)

Plan of Correction: The parent was contacted during the inspection and asked to bring the documentation.

Description: Based upon a review of files, one child's file does not contain documentation of immunization, prior to the first day of attendance. (Child A)
