Thank you for your time and assistance with the unannounced renewal inspection conducted today, June 15, 2015. The on-site visit was from 10: 45 a.m. to 1:00 p.m. On this date, at the time of arrival, there were twelve (12) children in the care of the provider and one assistant for a total of twenty-nine (29) points. There were a total of twelve (12) children enrolled in the program. There were five (5) children's records and four (4) staff records reviewed and updated. Presently, no medication is being administered. Areas of non-compliance may be found on the violation notice. Items observed and reviewed during the visit included: evacuation drills, emergency supplies, free play, TV time, lunch preparation, hand washing/ bathroom and diapering procedures, lunch, and nap time. The children are studying different countries in the world. Today's country was Japan and activities were implemented to increase children's cultural awareness. Interactions between the provider and children were positive and the children seem comfortable with the daily routine and schedule. This family day home is well equipped with a variety of age/stage materials for the children in care. Description: There was no documentation of Staff A obtaining an updated tuberculosis screening. Last documented update was 11/19/2012. Evidence: Based on record review and conversation with the provider it was determined an updated tuberculosis screening had not been obtained since 11/19/2012.

Plan of Correction: Staff A is scheduled to have a tuberculosis screening conducted tomorrow, 6/16/2015. Updates will be conducted two years from the prior date.
