**Discussed with the provider that she has a large amount of resilient material on the playground, but that she should move it around the fall zones, to help prevent injury.
An unannounced monitoring/renewal inspection was conducted today from 8:45am-10:30am. There were 8 children (17 points) directly supervised by 1 staff (a second provider with a preschooler was assisting the provider today). The home, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing freely and eating snack. Diapering procedures were also observed. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 10 calendar days from today. Evidence: 1) Reviewed 3 children's records and found that CH2 and CH3 did not have documentation of a second emergency contact. 2) Reviewed 3 children's records and found that CH3 did not have documentation of an address for their emergency contact.
