Comments:
An unannounced monitoring inspection was conducted today with the day home provider and her assistant. There were two resident children and nine daycare children in care for a total of 25 points. Children were observed playing with toy trucks, listening to a story, watching a cartoon, playing outside, and eating lunch. The lunch served today was pasta with tomato sauce and ground beef, pineapple, and milk. Three children's records were reviewed. There were no medications. The provider does not administer prescription and non-prescription medications. Violations:
Standard #: 22VAC40-111-60-A
Description: Based on documentation review, it was determined that not all children's records were complete. Evidence: Child A did not have the name, address, and telephone number of two emergency contacts.

Plan of Correction: The provider will get the two emergency contacts from the parent at pick-up.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to keep the play area outside in a safe condition. Evidence: There was a pick axe next to a tree stump adjacent to the play equipment; accessible to children.

Plan of Correction: The provider's husband will remove it to make it inaccessible to children.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface was no longer non-absorbent. Evidence: the diaper pad on the diaper table was torn in several places.

Plan of Correction: Provider will purchase a new diaper pad.
