Description: The provider failed to follow the manufacturer's instructions when a child was placed in a bouncy seat in the nap room to sleep. Evidence: Based on the physical plant review , a child (13 months)was found in the nap room by herself without direct supervision. The child was sleeping in a vibration bouncy seat that states that children must be directly supervised at all times. According to the assistant the child will cry if they put her in a crib.

Plan of Correction: We will make sure that a caregiver is in the room if she is sleeping in the bouncy seat, we will also work on her moving to a crib.
