Comments:
An unannounced monitoring inspection was completed on 7/15/2015. The inspector arrived at 10:35 A.M. and departed at 1:15 P.M. Eleven children were in care at 20 points with the licensee and one assistant caregiver. Three caregiver records were reviewed. Five children's records were reviewed. A physical plant walk-through was done.

Discussion: about stars rating system, ensuring that relatives that do not live in the household have a complete record when visiting.

Violations:
Standard #: 22VAC40-111-870-G
Description: Based on observation, the licensee failed to ensure that the tables and high chairs were cleaned after each use. Evidence: 1. The inspector observed the tables not being cleaned prior to eating lunch, the tables were played on during play time.

Plan of Correction: The licensee will ensure that the tables are cleaned with soap and water after each activity.

Standard #: 22VAC40-111-50-A
Description: Based on review of the attendance log and interviews with staff, the licensee failed to ensure that the log was completed. Evidence: 1. The inspector found that the attendance record was not completed at 10:57 A.M. Verified through staff interviews all 11 children were present by 9:30 A.M. The attendance record is to be completed within 30 minutes of the child's arrival.

Plan of Correction: The licensee will train the assistant to complete the attendance when the licensee is not present.

Standard #: 22VAC40-111-570-A
Description: Based on interviews with staff, the licensee failed to ensure that the provider does not exceed 16 points.

Evidence:
1. The inspector found through interviews that the assistant has been leaving at 1 P.M. for the past week during nap time and the second assistant arrives at 3 P.M. This leaves the licensee with over 16 points. Today the licensee had 20 points.

Plan of Correction: The licensee will rearrange staff's schedule to ensure that each caregiver does not exceed 16 points even during nap time.

Standard #: 22VAC40-111-680-A
Description: Based on observation, the licensee failed to ensure that the caregivers wash their hands with liquid soap and running water. Evidence: 1. The inspector observed the assistant caregiver using hand sanitizer after changing a diaper instead of using liquid soap and running water.

Plan of Correction: The licensee will remind staff of the proper procedures in changing a diaper.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the licensee failed to ensure that the diapering surface was cleaned and sanitized correctly. Evidence: 1. The inspector observed the assistant caregivers using bleach and water and wiping it off. The standard requires the diapering surface to be cleaned with soap and water and then to be sanitized and allowed to air dry.

Plan of Correction: The licensee will remind staff of the proper procedures in changing a diaper.
