Comments:
An unannounced monitoring inspection was conducted on 7/15/15 at 11:50 a. m. and concluded at 12:15. The inspector met with the provider and Family Day Home standards were reviewed. There were 11 children present with points equaling 32 with two caregivers present. The children had just finished lunch and were lying down for nap time. Three children records were reviewed. Three caregiver records were reviewed. The provider is not administering any medications at this time, and is not MAT (medication administered trained) Conversations were held with two caregivers/children were napping.

Violations:
Standard #: 22VAC40-111-180-A
Description: Based on review of an assistant record the provider did not get a Tuberculosis Screening updated within two years. Evidence: On 7/15/15, Assistant #1 had a Tuberculosis Screening dated 2/8/13 in the record. Tuberculosis Screenings should be updated every two years.

Plan of Correction: Will have Assistant #1 get a TB update.
