- Technical Assistance:
The backyard playground equipment shall remain off limits to children in care until the building materials surrounding the equipment are removed.
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 6 children, totaling 17 points. The children were observed playing with toys, listening to music, and interacting with each caregiver. This inspection was completed between approximately 8:45 am and 10:00 am. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include: the child's date of enrollment; the parent's signature on the "Authorization to Obtain Emergency Care" section of the registration form. 2. Child record #3 does not include: the parent's medical insurance policy identification number; the child's date of enrollment; the provider's signature indicating that she has viewed the child's original proof of identification; a shot record signed by the physician. Plan of Correction: The parents will be asked for the missing information for each record.
Standard #: 22VAC40-111-180-A Description: Based on a review of caregiver records, it was determined that one caregiver has not obtained a repeat TB Health Screening within the past 2 years. Evidence: The TB Health Screening for one assistant caregiver expired on 6/25/15. Plan of Correction: I will remind her to get a TB test done this week.
Standard #: 22VAC40-111-210-A Description: Based on a review of training documentation, it was determined that two caregivers did not attend at least 16 hours of training courses during the past year. Evidence: The provider and one assistant caregiver each attended a total of only 10 hours of training classes during the past year. Plan of Correction: We will both attend at least 16 hours of training this year.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that one item in the child care area has not been maintained in operable condition. Evidence: The diaper changing pad has ripped in various areas. Plan of Correction: I will get a new diaper changing pad.
Standard #: 22VAC40-111-460-J Description: Based on observations, it was determined that the provider did not remove all animal feces promptly. Evidence: There are two areas of dog feces in the backyard grassy area where the children play. Plan of Correction: The yard will be cleaned up before children go outside to play today.
