An unannounced monitoring inspection was conducted today, July 14, 2015. The violations from the previous inspection have been corrected. A violation notice was completed regarding two violations found during today's inspection. Upon arrival to the family day home the children were busy playing with the available toys. Lunch was served during the inspection. Nap time was also observed. There are 11 children in care totaling 19 points with three caregivers. Should you have any questions please do not hesitate to call. Inspection began: 10:40 am and ended 1:30 pm.

Standard #: 22VAC40-111-90-A
Description: Based on review of the children's files it was found that there is some information missing. Evidence: 1. Children's records #10 & 11 began care on 7/13/15 and there are no immunization records on file.

Plan of Correction: The provider will talk with the parent and have this information provided to her. The provider will ensure that copies of immunizations are received before a child may attend.

Standard #: 22VAC40-111-180-A
Description: Based on review of caregiver's records a staff person has a TB test that has expired. Evidence: 1. Staff #2 has a TB exam that has expired on 2/5/15.

Plan of Correction: Staff # 2 will have a TB exam. The provider will ensure that the TB tests are completed every two years.
