63.2 Liability Insurance Disclosure

An unannounced monitoring inspection was completed on 7/14/15. There were eight children present with the provider supervising. The point total was 11. Eleven children's records were reviewed for annual updates only. The inspector arrived at the provider's home at 1:45 pm and departed at 4:00 pm. The children were observed during nap and during snack. There was discussion with the provider about the following: updating background checks in the fall, annual training upcoming in the fall, fire drill for July, getting one child's school physical and immunizations when they are completed to enter public school.

Standard #: 22VAC40-111-180-A
Description: REPEAT VIOLATION Based on interview with the provider, the provider failed to ensure that the adult household member had a tuberculosis (TB) test within two years from the last test. Evidence: 1.The record of the household member was reviewed at the last inspection and the TB test was completed 10/5/12. The provider stated, at this inspection, that the household member has not updated the TB test.

Plan of Correction: The provider will have the household member get the TB test done this week or next. For future, the provider will ensure that the TB tests are updated before two years from the last.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider failed to ensure that all unused electrical outlets were covered with child-resistant protective covers. Evidence: 1.There were two unused and uncovered electrical outlets observed in the bathroom above the sink counter. 2.There was one unused and uncovered electrical outlet observed in the children's playroom under the wall heater near the floor level.

Plan of Correction: This was corrected during the inspection. The provider replaced the outlet covers that were on the sink counter and found a cover for the uncovered outlet in the playroom area. In the future, the provider will do a visual check daily to ensure that all outlets are covered.
