Technical Assistance:
All blankets shall be removed from the crib prior to it being used by the infant in care today.

Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for 2 enrolled children and two non-enrolled children who were visiting the home. The provider was caring for a total of 8 points of children today. The children were observed playing on indoor climbing pieces, enjoying creative play time with the use of the children's kitchen and food toys, and playing with cars. The children were to be served macaroni and cheese, steamed carrots, sliced grapes, wheat bread, and milk for lunch later today. This inspection was completed between approximately 9:00 am and 10:20 am. Thank you for meeting with me today. Violations:
Standard #: 22VAC40-111-800-B
Description: Based on a review of administrative documents, it was determined that the provider has not documented an annual review of her emergency preparedness plan. Evidence: The most recently documented review of the emergency preparedness plan was on 7/11/14.

Plan of Correction: I did review the plan earlier this month but didn't document the date. I will document the annual review.

Standard #: 22VAC40-111-50-C
Description: Based on observations, it was determined that the provider has not maintained the information in the children's records in a confidential manner. Evidence: The provider has posted a list of contact phone numbers for each enrolled child. The list is posted on the wall in the child care area.

Plan of Correction: I intentionally posted the list on the wall opposite the side of the room where parents come and go. I will also put a cover sheet on top of the list of phone numbers.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #2 does not include the parent's medical insurance policy identification number.

Plan of Correction: I already asked the parent for the information. I will ask again at pick up tonight.
