Technical Assistance:
An exit interview was conducted with the provider to discuss the results of today's inspection.

Comments:
An unannounced monitoring inspection was conducted today, July 22, 2015 to determine the provider's compliance with licensing standards. In attendance were 4 children totaling 12 points being supervised by the provider. Children were observed eating, sleeping on mats, and interacting with the provider. Children books, toys, and equipment were inspected and found to be age appropriate and in operable condition. The children's restroom was inspected with the required supplies present. All rooms of the home used by children were inspected with no hazards found. The outdoor playground and toys areas were inspected on this day. The family day home had the required postings located on the wall upon entering the home. The first aid kit, fire extinguisher, and emergency supplies were inspected with the required items present. Two children and the provider records were inspected with one violation found, see attached notice. The following was also inspected: daily attendance, fire drill log, shelter-in-place drills, emergency preparedness plan, parent agreements, and other pertinent information. Standard #: 22VAC40-111-180-A
Description: Based on the inspection of provider records, an updated TB test or screening was not present. Evidence: During the inspection of one provider record, a TB test dated 7/3/12 expired on 7/3/14. An updated TB test is required with documentation to review.

Plan of Correction: Per provider: I will have the staff to obtain an updated TB test or screening.
