An unannounced monitoring inspection was conducted on July 27, 2015 from approximately 11:20 am to 1:10 pm. Provider and two assistants were present with ten children. Children were observed during outside play, lunch and nap time. Two children?s records and two caregiver records were reviewed on this date. Licensing Inspector reviewed supervision, activities, equipment, diapering procedures, hand washing procedures, nutrition, children and caregiver records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Violations were reviewed with and verified by the provider during the exit interview on this date.

Standard #: 22VAC40-111-110-B
Description: Based on review, provider did not ensure 1 of 2 physical and immunization forms in children's records has a signature or stamp from the physician, physician's designee or local health department official. Evidence: On July 27, 2015, the record for child #1 (start date September 4, 2014) did not have signature or stamp on the physical and immunization record.

Plan of Correction: Provider will ask parent to contact the doctor and have it faxed with signature or stamp.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on review, provider did not ensure 1 of 2 caregivers has a updated background check completed within the past 3 years. Evidence: On July 27, 2015, the record for caregiver #1 had a most recent central registry check dated March 21, 2012 which is more than 3 years old.

Plan of Correction: A central registry check was completed and mailed.
