It is highly recommended that the provider remove some of the abundance of toys and materials in the home.
An unannounced monitoring inspection was conducted today. The provider was caring for 4 children, totaling 13 points. This inspection took place in the middle of the day. All children were napping during this inspection. The children had been served shredded chicken, rice, broccoli, mango, and milk for lunch earlier in the day. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include: a complete "Information for Parents" document. On this form, the provider has not included check in/out procedures, general daily schedule, discipline policies, and termination policies. 2. Child record #2 does not include: each parent's work address; a physical exam report; a signed "Provisions of the Emergency Plan" document; a complete "Information for Parents" document. On this form, the provider has not include check in/out procedures, general daily schedule, and termination policies. Plan of Correction: The forms will be updated and each parent will be asked for the missing information.
Standard #: 22VAC40-111-240-A Description: Based on observations, it was determined that the front of the home was not maintained in a safe condition. Evidence: Upon walking up to the front door, the Inspector observed a hedge trimmer and a lawn edger on the ground at the bottom of the step at the front door. Plan of Correction: The pieces of equipment were moved to the garage during the inspection.
Standard #: 22VAC40-80-120-E-2 Description: Based on observations, it was determined that the provider did not post the findings from the most recent inspection. Evidence: The most recent inspection of the provider's family day home took place on 2/23/15. The provider did post the violation notice for this inspection. Plan of Correction: I will post the violation notice for today's inspection.
