Discussed with Provider: using the DSS model forms and training hour requirements for annual training. Hours will be adjusted as well as the requested age range.
An unannounced renewal study was conducted today. There are 9 children present (24 points) with 3 adults. One adult and one child's file were reviewed. See violation notice for missing information. Provider's first aid certification will expire in September - a class has already been scheduled, prior to expiration. First aid supplies, emergency supplies and emergency escape drills are all in compliance. Children were observed on a walk in the neighborhood, playing with a variety of toys, washing hands and enjoying morning snack. Today is one child's first day. The child is quietly observing all the activity around them. Provider is in the process of obtaining her CDA. Due to case load adjustments, your next inspection will be with a different inspector. Standard #: 22VAC40-111-60-B Description: Based upon a review of files, one child's file is incomplete. Evidence: Child A's file is missing: insurance information and policy number, Emergency Medical authorization. Plan of Correction: I will ask the parent to provide the information tonight.
