Technical Assistance:
"Cleaned" means treated in such a way as to remove dirt and debris by scrubbing and washing with soap and water or detergent solution and rinsing with water. "Sanitized" means treated in such a way as to remove bacteria and viruses from inanimate surfaces through first cleaning and secondly using a solution of one tablespoon of bleach mixed with one gallon of water and prepared fresh daily or using a sanitizing solution approved by the U.S. Environmental Protection Agency. The surface of the item is sprayed or dipped into the sanitizing solution and then allowed to air dry. Please be sure all substitutions to the menu are update promptly. Consult provided on the stairs/gates.

Comments:
An unannounced monitoring inspection was conducted with the provider beginning at 9:30 a.m. to 12:00 p.m. There were two children in care ranging in age from eighteen months to three years for a total of five points. Children were observed watching an age appropriate cartoon program on television and having free play with toys. Diapering procedures were observed. The daycare area was observed. Children were observed having morning snack. The three year old had a cheese stick and milk and the eighteen month old had mini pancakes and apple juice. One provider record and three household records were reviewed and complete. Two children's records were reviewed. Standard #: 22VAC40-111-960-G
Description: Based on observation, the provider heated a bottle of milk in the microwave.

Plan of Correction: The child was crying and I was trying to get the bottle ready. I don't usually use the microwave and have a bottle warmer. I didn't feed the child the bottle after microwaving it.

Standard #: 22VAC40-111-90-C
Description: Based on review of documentation, the provider failed to obtain documentation of additional immunizations as required. Evidence: Child A, date of birth 2/25/2014, last documented immunizations were dated 5/1/2014. Immunization documentation is required to be updated every six months for children under two years.

Plan of Correction: Updated immunizations will be asked for from the parent at pick up.

Standard #: 22VAC40-111-430-A
Description: Based on observation, a child under the age of two years of age had access to stairs. Evidence: The gate on the basement stairs to the child care area was tied open with a white string. Child A, age 18 months, climbed the stairs.

Plan of Correction: A new gate will be installed. The current gate will be shut while the children are in the child care area.

Standard #: 22VAC40-111-430-B
Description: Based on observation and interview with the provider, it was determined that the gate used as a protective barrier on the steps in the child care area was pressure mounted. Evidence: A "Summer" brand pressure mounted gate was observed on the fourth step on the stairs to the child care area in the basement.

Plan of Correction: A new gate will be purchased and installed.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider failed to ensure that all children's hands were washed as required. Evidence: Child A's hands were not washed after diapering. Child B's hands were not washed before being served snack.

Plan of Correction: Children will wash their hands after diapering and before snack.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to clean and sanitize the diapering surface after diapering. Evidence: After diapering a child, the diapering surface was not cleaned or sanitized.

Plan of Correction: The diapering pad will be cleaned and sanitized after use.
