54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced renewal inspection was initiated today at 11am. There were 7 children (plus the Provider's 2 children), totaling 18 points, under the supervision of 3 caregivers. During the inspection, children were observed coming back from a trip to the library, enjoying free play, cleaning up for and being served lunch. The children had pasta with vegetables in a tomato sauce, strawberries and milk. The Provider is certified to administer medications (MAT certified). However, there are no children with any medication needs during program hours. Should you have any questions, please contact me.
Standard #: 22VAC40-111-90-C Description: Based on file review, 1 out of 6 children's file does not have additional immunizations every six months up to the age of two years. Evidence 1. Child #4 is 2 years and 8 months old. The most recent immunization record was completed when the child was 15 months old. Plan of Correction: The Provider believed this was given to her. She will continue to look for this. If it is not found, she will ask the parent for a copy to keep on file.
Standard #: 22VAC40-111-140-B-3 Description: Based on interview and file review, caregiver #1 does not have a current certification in CPR. Evidence 1. Caregiver #1's CPR certification expired on 4/13/15. Plan of Correction: We are scheduled to get recertified this weekend.
Standard #: 22VAC40-111-140-B-4 Description: Based on interview and file review, caregiver #1 does not have a current certification in First Aid. Evidence 1. Caregiver #1's First Aid certification expired on 4/13/15. Plan of Correction: We are scheduled to get recertified this weekend.
Standard #: 22VAC40-111-150-D-2 Description: Based on interview and file review, caregiver #2 does not have a current certification in First Aid. Evidence 1. Caregiver #2's First Aid certification expired on 4/13/15. Plan of Correction: We are scheduled to get recertified this weekend.
Standard #: 22VAC40-111-290 Description: Based on observations, there were sharp objects in an accessible area to children. Evidence 1. In the basement bathroom, there was a pair of scissors in one of the drawers. Plan of Correction: This will be moved to an area inaccessible to the children.
