54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring visit and home tour was conducted from 12:44 pm-2:10 pm. At the time of the inspector's arrival there were 7 children ages 13mos-9yrs, totalling 14 points in care with the licensee. Children were observed participating in free play, lunch, nap, diapering and handwashing. A sample of 3 children's records and 1 staff record and 1 household member record were reviewed. Medication is not administered at this site. There were no reported injuries since the last inspection. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
Standard #: 22VAC40-111-240-A Description: Based on observation it was determined that the provider did not ensure that all areas and furnishings of the family day home, inside and outside, shall be maintained in a safe condition. Evidence: The swings utilized by the children had rusted chains and the eye bolts used to suspend the swings from were rusted and showing wear. Plan of Correction: The swings and the bolts will be replaced.
