An unannounced renewal inspection was conducted with provider and assistant. There were eleven day care children in care and two resident children, ranging in age from 7 months to five years old totalling 31 points. The children were having rest time. Two new children's files were reviewed - please see Supplemental Summary for missing items. Provider does not administer medication - Medication Administration Training is required to administer medication. Provider's first aid and CPR certification is valid through 10/19/2015. Description: Based on review of children's files, the provider failed to maintain a complete record for each child. Evidence: Child A and B did not have the address for two emergency contacts or signed authorization to get emergency medical care if necessary.

Plan of Correction: Provider will request the information from the parents again.
