Initial Licensing Inspection was conducted beginning 9:05 and ending at 10:45. There were 4 children present ages two - nine. The children were observed playing and interacting. The whole house was inspected. Family and children files were reviewed - see violation notice. Emergency Escape drills will be started this month. Menu was posted. Attendance sheet is complete. Standard #: 22VAC40-111-60-B Description: Based on a review of 5 children's records, it was determined that 3 files did not have complete information. Evidence: The file for Child #1 and #2 did not have medical insurance policy number and also does not have street addresses for emergency contacts. The file for Child #5, who enrolled on 6/1/15, does not have a physical on file. Plan of Correction: The provider will get the required information from the parents.
