54.1-3408 Provider must be MAT certified to administer prescription medication.

An unannounced monitoring inspection was conducted today. The provider was caring for 5 children, totaling 15 points. The children were observed playing with toys, engaging in creative play activities, and watching a children's television show. The children were to be served ground turkey, rice, broccoli, bananas, and milk for lunch later today. Thank you for meeting with me during this inspection. Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that one record was not complete. Evidence: Child record #3 does not include the full name of the second emergency contact person.

Plan of Correction: I will ask the parent for the information at pick up tonight.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that the provider did not completely enclose the child care area in a safe manner. Evidence: The media/exercise room is adjacent to the child care area. Inside the exercise room is a large treadmill that cannot be used by the children. The door to this room was open when the Inspector arrived today.

Plan of Correction: The door was closed during the inspection. I will keep the door closed at all times.

Standard #: 22VAC40-111-430-B
Description: Based on observations, it was determined that the provider has a pressure mounted gate in place at the stair opening leading to the upper level of the deck. Evidence: The children play on the lower level of the deck, which is at ground level. However, the opening to the full set of stairs leading to the upper level of the deck is in the corner of the children's play area. A pressure mounted gate has been installed at the opening to the full set of stairs.

Plan of Correction: I will replace the gate with a hardware mounted gate.
