54.1-3408 Provider must be MAT certified to administer prescription medication.
3 children and the provider was present during this monitoring visit. A total of 6 points. The children are playing with blocks, doing puzzles and counting different animals. The provider is teaching Spanish and English during the whole visit. Lots of conversations and interactions in both languages. The provider has lots of supplies for the children to play with. A large back yard is also set up for play. This visit was started at 10:25 and ended at 11:45. Please call me at 703-203-8659 if you have any questions regarding this visit.
Standard #: 22VAC40-111-110-A Description: The provider failed to get the Virginia health forms for 2 children. Evidence: Based on the review of the three children's files , 2 children, child A and C, are missing the physical form. The provider stated that she has asked for this but received the notes from children's well visits. Plan of Correction: I will make sure I show the form I need to the parents and ask them to bring it to me as soon as possible. I have asked them before and the gave me what is in the file.
Standard #: 22VAC40-111-60-B Description: The provider failed to get the addresses for the emergency contact for one child. Evidence: Reviewed the files of 3 children, one child ,child C, is missing the addresses for the two emergency contacts. Plan of Correction: I will ask the parents for the information
