Discussed with Provider that all original background checks (Criminal record and Central registry check) are required to be kept on-site.
Today an unannounced renewal inspection was conducted with the Provider-Mrs. Grace. This visit took place in the morning between 9:30-12 there were 7 children with the Provider and 1 assistant for a total of 22 points within the supervision guidelines. The children were observed watching TV, having outdoor play time, snack, morning nap, lunch and in transition for an afternoon nap. Lunch today consisted of baked chicken, carrots, rice and milk. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, menu and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-960-E Description: Based on observation of the bottles, one bottle was found without the name and date. Evidence: One infant bottle was found without name and date. Plan of Correction: Provider stated that she will make sure all bottles are labeled and dated.
Standard #: 22VAC40-111-120-B Description: Based on observation of the provider records, the assistant record did not include documentation of two or more references were verified. Evidence: Caregiver #2 record did not include documentation of one of the references. Plan of Correction: Provider stated that she has known the provider for 40 years. Provider will have them submit another reference.
Standard #: 22VAC40-111-180-A Description: Based on observation of the caregiver and household records, two of the records did not include documentation of a repeat TB screening two years from the date of the last screening. Evidence: Household member #1 & #2's record did not have an updated TB screening since 4/13. Plan of Correction: Provider stated that she previously had a positive Tb/chest x-ray and was told by her doctor that she never need to have another screening. Provider will have a screening conducted with her physician.
Standard #: 22VAC40-111-540-A Description: Based on observation of the children, two children were observed sitting in a infant rocking/chair not being used according to the manufacturers instructions and with a safety strap. Evidence: Two children over 12 months of age were observed in a infant chair without a safety strap and over the weight requirement. Plan of Correction: Provider stated that the children like climbing in and sitting in those chairs when they want to rest after morning snack. Provider will remove chairs from the play area when not being used by smaller/infants.
Standard #: 22VAC40-191-40-D-1-c Description: Based on observation of the provider records, two of the record did not include documentation of a repeat criminal record and central registry check three years from the date of the last screening. Evidence: Household member #1 & #2's record did not have documentation of criminal record and central registry checks. Plan of Correction: Provider stated that she should receive them any day, she submitted them 6 weeks ago.
Standard #: 22VAC40-191-50-A-3-a Description: Based on observation of the Provider records, one of the records did not include documentation of an original criminal record and central registry check. Evidence: Caregiver #2 record did not include original background checks. Plan of Correction: Provider stated that she gave the originals to her Arlington County worker. Provider was informed that all original documents should be kept on-site.
