Comments:
Conducted an unannounced monitoring study with the provider, an assistant, and 10 children for a total 26 points. The ages of the children range from nine weeks-old through seven years-old. The children were eating lunch and napping during the inspection. One household record and five children's records were reviewed during the inspection. The provider and the assistant are current in First Aid and CPR. Start time: 10:50am End time: 12:50pm

Standard #: 22VAC40-111-60-B
Description: *Repeat Based on a review of five children's records, it was determined that one record was missing information. Evidence: Child A's record was missing the street addresses for two emergency contacts.

Plan of Correction: The provider stated that she will get the missing information from the parents today.
