Technical Assistance:
Standard 750-A: sunscreen and other topical ointments was discussed with the substitute provider. It is recommended that sliding gates that pose pinch hazards are not used in the family day home.

an unannounced monitoring inspection was conducted from 10:00am-11:35am. upon arrival there were 9 children in care totaling 25 points with 3 caregivers. Five children and all household/caregiver records were reviewed. No medication is administered at this time. Children were observed playing with caregivers. One baby was observed sleeping in a swing.

Standard #: 22VAC40-111-60-B
Description: Based on a record review the licensee failed to ensure all children's records contained the required information. Evidence: Child #3 and #4 did not have documentation of a second emergency contact address.

Plan of Correction: The licensee will obtain this information from the parents at pick-up and document it in the record.

Standard #: 22VAC40-111-280
Description: Based on observation the licensee failed to ensure potentially hazardous substances were inaccessible to the children in care. Evidence: In the bathroom used by the children there was a liquid deodorizer plugged into the wall outlet near the light switch. The plug-in was in reach of the children.

Plan of Correction: The plug-in was removed during the inspection.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview with the substitute provider, the licensee failed to ensure there was an operable landline telephone (exluding a cordless) available.

Plan of Correction: The substitute provider was unable to locate a landline telephone during the inspection. The only telephone she was aware of was the cordless.

Standard #: 22VAC40-111-530-H
Description: Based on observation the licensee failed to ensure when pillows are assigned for individual use they were covered with pillow cases. Evidence: There were 2 cots set-up for nap time. Both of these cots had pillows with no cases.

Plan of Correction: The substitute provider removed the pillows during the inspection. They will be replaced wit individually assigned pillows with cases.
