Licensing Inspector arrived at 9:45 and departed at 11:00 AM to investigate a complaint received in the Eastern Licensing Office. Caregivers were interviewed and records reviewed regarding allegations of inadequate supervision and care of children. No evidence was found to support the allegations. So the complaint is deemed to be not valid. However, there were violations found in the areas of administration and transportation.

Description: Based on a review of one child's record, it was determined that the licensee did not ensure that general written permission was obtained from the parent of each child for the provider to take the child off the premises of the family day home. Evidence: 1. Provider stated that this summer she transported child #1 approximately four blocks from the family day home to a neighborhood school so that child #1 could catch the bus to summer school. 2. Provider did not have written parental permission to transport child #1 on file.

Plan of Correction: Provider will ensure that when children are to be transported that written parental permission is obtained.

Description: Based on a review of one child's record, it was determined that the licensee did not ensure that that before the child's first day of attendance parent's written acknowledgement of the written information for parents was obtained. Evidence: The "Information for Parents" and "Provisions of the Emergency Preparedness and Response Plan" documents in the record for child #1, date of enrollment 6/15/15, were no signed by the parent.

Plan of Correction: Provider will ensure that all documents are signed by parents prior to the child beginning attendance.
