Technical Assistance:
1. Some of the decals on the glass doors should be lowered to the eye level of the younger children.

Comments:
Conducted an unannounced monitoring inspection. Observed 7 children = 15 points with the Provider + Assistant. Ratios were in compliance and supervision was constant. Children were being diapered and taking naps during the inspection. The children played outside this morning. The daycare areas of the home were clean, safeguarded appropriately, and sufficiently supplied with toys and equipment for the children. The Provider reported lunch included: tuna fish, bread, green beans, oranges, and milk. No medications reported to be on-site. Emergency preparedness supplies were in order. First Aid + CPR training current. Required documents were posted. Record keeping and training violations found. Inspection completed 12:10-1:15pm. Description: Based on children's records reviewed, it was determined that 1 of 9 records did not include the following information. Evidence: Child #11 - 1 of 2 emergency contact addresses, medical insurance policy number, and the parents signed acknowledgement of receipt of policies/procedures listed on the Information for Parents form.

Standard #: 22VAC40-111-210-A
Description: Based on records reviewed, it was determined that a caregiver did not obtain a minimum of 16 hours of annual training. Evidence: Assistant #1 - For the period of February 2014-January 2015 a total of 2 hours of annual training was obtained.
