54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced inspection was conducted beginning at 1:45 p.m. and ending at 3:00 p.m. with the provider and assistant. There were four children in care ranging in age from eleven months to four years of age for a total of eleven points. The provider was providing substitute care for three of the children and they were picked up during the inspection. An infant was observed waking and being fed a bottle. Two staff records and two children's records were reviewed. Emergency drills were up to date and posted. Standard #: 22VAC40-111-920 Description: Based on observation, the weekly menu was not complete for 9/28/2015-10/2/2015 . Evidence: 9/30/15-10/2/2015 were not documented on the menu. Plan of Correction: The menu will be updated with the dates.

Standard #: 22VAC40-111-240-A Description: Based on observation, not all areas of the family day home were maintained in a safe condition. Evidence: There is peeling paint on the railing on the stairs and the carpet on the stairs is worn in several areas creating a tripping hazard. Plan of Correction: The railing will be repaired by next week. The carpet will be repaired or removed. No children will be allowed to use the stairs without supervision.
