An unannounced monitoring inspection was conducted from 10:30 a.m. - 12:20 p.m. There was one child in care with the provider. One child's record was reviewed. The record for the provider was reviewed. The child in care was observed playing, talking and babbling, being cared for and comforted by the provider, during hand washing, lunch and watching a child's program. Standard #: 22VAC40-111-90-C Description: Based on documentation the facility failed to have documentation of additional immunizations once every six months for a child under the age of two years. Evidence: 1. On 10/1/2015 the licensing inspector reviewed the immunizations for child #1 and there was no documentation of additional immunizations once every six months for the child under two years old. 2. On 10/01/2015 the provider confirmed there was no documentation in the record for child #1 of additional immunizations. Plan of Correction: I notified the parent immediately and she will get this information to me for the record.
