54.1-3408 Provider must be MAT certified to administer prescription medication.
- Technical Assistance:
The caregivers need 16 hours of annual training each year.
An unannounced inspection was conducted from 10 Am until 11 15 AM. There were 7 children present with the Provider and assistant for a total of 22 points. According to the Provider, another two children will arrive from half day Pre K program. Staff files and two children's files were reviewed and the Provider knows CPR will need to be renewed by January, 2016. The Provider has cribs available to the children.
Standard #: 22VAC40-111-120-B Description: Based on record review and staff interviews, the Provider does not have a complete file for Caregiver #2. Evidence: 1. The Provider stated that caregiver #2 does help out with ratios and the Inspector reviewed the file. It did not contain 2 references and documentation of Orientation. Plan of Correction: I will go over this information with her.
Standard #: 22VAC40-111-90-A Description: Based on record review of two new children's files, the provider has not received an immunization record for child #1. Plan of Correction: The child just started today and the parent will be ringing in the completed form.
