Comments:
Today an unannounced monitoring visit was conducted beginning at 11am. There were 8 children (total of 24 points), including the Provider's own child with the Provider and assistant caregiver. New caregiver records, current ones and 6 new children's records were reviewed. There are currently no medications for any children enrolled at this time. Annual training hours were also reviewed and current. Children were observed finishing lunch, having quiet time, and preparing for nap. The program continues to offer a variety of age appropriate activities for all ages. This inspection, including the exit interview, ended at 12:10pm. Should you have any questions, please call.

Standard #: 22VAC40-111-60-B
Description: Based on record review, one child's record (#2) does not have complete required documentation. Evidence: Child #2 has a start date of 8/24/15. There is no documentation of a physical examination in their record within 30 days of their start date.

Plan of Correction: I thought I had this. I will ask the parents to get this documentation for me.

Standard #: 22VAC40-111-180-A
Description: Based on record review, two adult records (#4 and 5) do not have updated TB tests within 2 years of the most recent results. Evidence: Adult records #4 and 5 last obtained a TB test in August, 2013.

Plan of Correction: We will renew these immediately and every two years.
