An unannounced monitoring inspection was conducted today from approximately 1:30 to 2:30pm. Eight children, including the substitute provider's own child, were present with the substitute provider and assistant. Total 25 points with two caregivers. The provider is currently out of town for about two weeks. The children were all resting at the time of the inspection. Two new children have been enrolled since the last inspection. Their records were reviewed. The outside play area was not checked today because of rain. If you have any questions, I can be reached at 703-479-4709.

Description: Based on observation, a working battery-operated weather band radio was not available. Findings: At the time of the inspection, the inspector found that there were no batteries in the portable radio, and no batteries could be located to put in the radio.

Plan of Correction: Batteries for the radio will be obtained, as well as extra batteries, so that batteries are available at all times.

Description: Based on observation, bottles for the infants were not labeled with the child's name and date.

Plan of Correction: All bottles will be labeled with the child's full name and date.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on record review, a CPS check of the central registry was not obtained for a new assistant who began employment on August 19, 2015.

Plan of Correction: The substitute provider stated the request for the check was sent and then returned by CPS, because it was the wrong form and the wrong amount of payment. The request has been re-sent on the correct form.
